Session Type (Concurrent, After 5, or Multi-Session)

Presentation Title (Maximum 50 characters, including spaces)

Presentation Description (Maximum 500 characters, including spaces) if selected, this is what will
be included in the preliminary and on site programs.

Division, Forum, Interest Group (Is your session under support of a division, forum, or interest
group? You must have this approved by a division, forum or interest group prior to submitting.)

Presentation Level (Basic, Intermediate or Advanced)

Focus Area (Quality Basics, Customer Service, Driving Quality Throughout the Organization, Improved Performance, 21st Century Needs/Competencies/Issues)

Presenter Information (Maximum of two presenters – Name, Company Affiliation, address, phone, e-mail, ASQ member number)

Biography (Short introductory bio – maximum 1000 characters, including spaces)

Who Should Attend Your Session (Describe your ideal attendee – maximum 200 characters, including spaces)

Paper Abstract (Minimum 10,000 characters - Maximum 20,000 characters, including spaces – about 3-6 pages) Sessions will be reviewed and selected on the contents of this Paper Abstract. Please align the length of all submissions to the aforementioned guidelines to ensure that an accurate and complete description of the session can be rendered. You will be required to copy and paste your paper abstract into the session application. All text abstract - No graphs, tables, or charts allowed in paper abstract.
